Community Day Care

The Community Day Care in the Health and Wellness Centre offers older people meaningful therapeutic activities with a person-centred focus. The service is open Monday to Friday from 9.00am to 3.00pm. The service is based on a social model and the referral process is led by the multidisciplinary team, linking with the local public health nurses, general practitioners, the critical care team in TUH and the RANP/CRISP program. The service supports older persons who are living at home in the local communities, across the CHO7 catchment area (Lucan, Under Health and Wellness

The Day Service provides a social environment for people who can be isolated and housebound but want to age in place by focusing on their wellbeing and preventing deterioriation with health supports from public health. People enjoy a fun activity day program with a focus on health and wellbeing.

Peamount Healthcare also provides weekly Meals on Wheels, to an average of 420 people living alone in the local communities of Rathcoole, Lucan, Newcastle and Clondalkin.

Tanya has extensive experience working in senior leadership roles in the health service. Following a period as Director of Nursing and Deputy CEO in the Mater Misericordiae University Hospital, her most recent post has been Deputy Chief Nursing Officer in the Department of Health. Tanya has also worked in St Vincent’s University Hospital and the Office of the Director General in the Health Service Executive. Tanya qualified as a nurse, completing a post graduate BNS, Diplomas in Oncology , Quality and Leadership and has an MSc in Healthcare Management. She is a Fellow of the Faculty of Nursing of the Royal College of Surgeons of Ireland. Tanya is committed to ensuring the delivery of high-quality person centered health care and looks to build strong relationships with all stakeholders to deliver innovative and sustainable service improvement.

Catherine Slattery

Director of Rehabilitation Services

Catherine has extensive experience of working in the health and social care sector. She qualified as both a general and psychiatric nurse and worked in the NHS and Ireland in various nursing roles. In the last 14 years she has held a number of senior health and social care positions in the voluntary and community sector. She holds a BA in Psychology, MBA in Health Services Management, post graduate qualification in Risk Management and a black belt in Lean Healthcare. In recent years Catherine’s special interests are in the areas of Rehabilitation and Disability.

Elizabeth McGrattan

Director of Nursing and Social Care

Elizabeth has 15 years nursing experience in both the acute, social care, private and community sector. She qualified in general nursing and has worked in various roles including acute respiratory care, disability services and community care.

Elizabeth joined Peamount Healthcare in 2019 as Clinical Nurse Manager in Practice Development and more recently as Quality and Continuous Improvement Manager. Elizabeth was appointed to the role of Director of Nursing and Social Care in April 2023.

Elizabeth holds a BScN in General Nursing from Trinity College Dublin, a Post Graduate Diploma in Adult Respiratory Care from the University of Limerick and is currently undertaking a Post Graduate Diploma in Quality and Safety in Healthcare Management with the Royal College of Surgeons Ireland.

Elizabeth has a special interest in quality and safety and working closely with the nursing and social care teams to ensure positive experiences for all who access services in Peamount Healthcare.
